Support after hospital discharge
Moorland Community Care Group was formally constituted in 2016 following the success of its original incarnation as Bovey Community Care. Our area of operation covers the Dartmoor towns of Bovey Tracey, Ashburton and Buckfastleigh and surrounding parishes.
We operate through three locally manned organisations: Bovey Community Care, Ashburton Community Care and Buckfastleigh Community Care, each of which is specific to that town and supported by local volunteers.
Moorland Community Care Group is a part of the Southern Devon Wellbeing Partnership which also includes several local organisations with similar remits. We provide Wellbeing services to patients registered at local surgeries enabling them to make improvements in their own lives and to take control of their health conditions.
